An important and stressful day in the life of hospital administrator Dr Lisa Cuddy begins at 5am with her routine interrupted by her feverish baby. Eventually handing over to her nanny and dealing with a demanding Lucas, Cuddy reaches Princeton Plainsboro knowing she has to make a crucial deal with medical insurer Atlantic Net, but she's faced with various other problems before she's even reached her office...

Double the ordered amount of ephedrine has been delivered to the pharmacy and there's trouble in the operating theatre where Dr Houdrain, freezing and mid surgery, complains that House has had the air conditioning turned up to hurry him along.

She's also confronted by a cancer sufferer who's demanding human breast milk as part of his treatment and a patient who's suing the hospital for reattaching his severed thumb against his wishes. Then a fight breaks out between the surgeons in pre-op, and the board tells Cuddy: no insurance deal, no job!
