Millionaire philanthropist Benjamin Byrd collapses after making an extremely generous donation and is rushed to Princeton Plainsboro. House believes his altruistic behaviour is a symptom of a deeper disorder but when he struggles to find an explanation he convinces former prison doctor Jessica Adams to contribute her expertise to the investigation.
Byrd explains that he keeps only enough money to survive on and is in the process of giving the rest away, a philosophy that's already cost him his family. Adams thinks he's just really generous but Parks believes he may have a neurological problem.
House is hoping to get his old team together and has been trying to contact Thirteen. She explains that she's happy in her new life with her girlfriend and doesn't want to come back, but House makes her feel guilty and persuades her to return.
The team's diagnosis changes to Whipple's Disease but House still isn't sure. Byrd makes more generous gestures, offering to fund House's team and even deciding to donate a kidney to one of Wilson's patients. Foreman authorises the donation even though House claims Byrd is not in his right mind, but the operation is cancelled after House gives him something to make him unstable.
Thirteen pretends that she is also in need of a kidney and Byrd offers to donate to her, too, claiming he can live on dialysis - evidence, if they needed it, that something is wrong. House now suggests Porphyria.
Later, while talking to Wilson, House finally understands the problem. Byrd has Plummer's Disease and can be cured by the removal of a nodule on his thyroid. After the operation, Byrd is a normal person again and intending to reunite with his family.
Meanwhile, House watches Thirteen leave with her girlfriend, calls her back in and fires her, thereby giving her permission to enjoy what life she has left to her...
