A number of strange and puzzling deaths have Sam and Dean dumbfounded. The weirdness continues when they find Bobby is also married to Ellen (who actually sacrificed her life a year beforehand.
Tracing the family tree of the victims, Ellen discovers their ancestors all came to the US in 1912 aboard the Titanic (which none of them have heard of). Sam then finds a photo and recognises the ship's first officer as Balthazar.
Balthazar is summoned and admits he saved the ship because he hated the movie, and that the chain reaction is the reason why Ellen is now alive. The Winchester boys then discover the deaths are the work of Atropos, one of the Fates sisters in Greek mythology.
She then sets her sights on Sam and Dean for cheating death once too often, but Castiel saves then and confronts Atropos, who gets Castiel to admit it was he who convinced Balthazar to save the Titanic in the first place (so that he could have 50,000 more souls in his war against Heaven).
The episode ends with Castiel being convinced into restoring the timeline, which means no more Ellen or Jo.
